Registration Process
In order to completely register, you must have all of the following:
1) A copy of the player's Birth Certificate, or other proof of age (passport) – this is for new players only, not returning players. Please do not bring the original, only a copy.
2) A completed Medical Form (above) dated after January 1, 2008, signed by a doctor certifying that the player is physically fit to play tackle football by May 15th, 2008.
3) A completed Registration Form (above). This needs to be signed by both a parent and by the player.
4) A small photograph of their head and shoulders only, (NO HAT or CAP) 2"x 2" or 2"x 3". A school picture works best.
5) A copy of their school report card for ALL of the 2007-2008 school year grading periods. It has to be for the entire school year.
6) Cash, credit card, or a check made out to the "Bayside Broncos" for $325.00 (If you are registering more than one player, please deduct $25 from each registration - 2 players = $600, 3 players = $875)
(NOTE: Fees are non-refundable after the player participates in a practice.)
